Dental Services
Patient Responsibilities
As a patient, you agree to:

- Follow pre- and post-treatment instructions
- Attend scheduled appointments or provide adequate notice of cancellation
- Pay for services as agreed
Our Responsibilities

Informed Consent
Before treatment, we will explain the proposed procedure, discuss risks and benefits, present alternatives, answer your questions, and obtain your consent. You have the right to ask questions and decline treatment.
Appointments
Scheduling
We'll work with you to schedule convenient appointments. We recommend booking in advance for non-urgent care.
Cancellation Policy
We request 24-48 hours notice for cancellations or rescheduling. This allows us to offer the appointment to another patient.
Repeated no-shows or late cancellations may result in:
- Cancellation fees
- Requirement for deposits
- Dismissal from the practice
Late Arrivals
Arriving late may shorten your appointment time. Significant delays may require rescheduling.
Emergencies
We make every effort to accommodate dental emergencies. Call us immediately if you have a dental emergency.
Payment Terms
Payment Due
Payment is due at the time of service unless other arrangements have been made.
Insurance
We will submit claims to your insurance on your behalf. However:
- You are responsible for knowing your coverage
- Insurance estimates are not guarantees
- You are responsible for any balance not covered by insurance
Payment Methods
We accept:
- Cash
- Debit
- Major credit cards (Visa, Mastercard, American Express)
- Insurance assignment (direct billing)
Collections
Accounts not paid within 90 days may be subject to collection efforts. Collection costs may be added to your balance.
Liability
Limitation of Liability
BluSky Dental, its dentists, and staff are not liable for:
- Outcomes that are not within our control
- Complications arising from patient non-compliance
- Indirect or consequential damages
This does not limit liability for professional negligence or malpractice.
Indemnification
You agree to indemnify BluSky Dental against claims arising from your breach of these Terms.

If you have a concern:
- Speak with your treating dentist
- Contact our office manager
- Submit a written complaint
We take all concerns seriously and will respond promptly.
